1. Our Services

FormsVault provides an AI-powered Google Forms™ Add-on that functions as a form and quiz builder utilizing artificial intelligence to generate online surveys, forms, assessments, and quizzes directly inside your regular Google Forms™ workspace. Our features include browser-based interaction tracking (AI Quiz Monitor) and form response-based automated data summaries (Quiz Insights / Form Insights).

2. Billing via Lemon Squeezy

2.1 The Merchant of Record

FormsVault subscriptions and AI Credits packs are sold directly by LemonSqueezy.com, the official Merchant of Record (MoR). As the official global reseller of FormsVault, they directly handle the payment processing for all purchases, subscription billing, renewals, as well as fraud prevention, international and localized tax processing. Lemon Squeezy is an independent financial entity and is neither a subsidiary, a related corporate entity, a distinct operational branch, nor a distinct person for FormsVault.

2.2 Your Bank or Card Statement

When you buy a FormsVault subscription or an AI Credits pack, the charge will show up clearly on your credit card or bank statement under the payment descriptor: LEMONSQ* FORMSVAULT

2.3 Subscription Plans, Renewals and Tier Changes

FormsVault offers a Free tier with limited monthly generation. Paid subscriptions (FAST, FORWARD, and FOCUS tiers) automatically renew at the end of each monthly or annual cycle. You may request tier modifications at any time via your Lemon Squeezy Customer Portal:

• Upgrades: Upgrades between Billing Plan tiers or billing frequencies (Monthly Plan to Annual Plan) take effect immediately for all active subscriptions in the account upon successful payment confirmation via Lemon Squeezy. Any applicable prorated charges for the remainder of the billing period will be calculated and processed automatically by Lemon Squeezy.
• Downgrades: Downgrades between plan tiers or billing frequencies take effect for all active subscriptions in the account only at the end of your current active billing cycle. You will retain full access to your existing tier features and AI Credit allocations through the end of the paid period.
• Cancellation: You can easily cancel future subscription renewals at any time right inside your individual Lemon Squeezy Customer Portal before the start of your next billing cycle.

2.4 Price & Feature Adjustments

We want to constantly improve FormsVault. We reserve the right to update our subscription pricing configurations, credit tier packages, and top-up costs over time. Don't worry—we will always give you at least thirty (30) days' advance notice via email or a dashboard notification before any recurring price change happens. Your continued use of our platform after this notice interval constitutes agreement to the updated pricing structure.

3. FormsVault AI Credits

3.1 Subscription Plan AI Credits

Paid subscription tiers come with a fixed allocation of FormsVault AI Credits per billing cycle. Unused AI Credits expire automatically at the end of each billing cycle and do not roll over to the next period.

• Upon an immediate Upgrade confirmation to a new Billing Plan, your new tier AI Credits allocation is issued instantly upon Lemon Squeezy payment confirmation, resetting your AI Credits cycle to match your updated billing plan.
• Upon a scheduled Downgrade confirmation to a new Billing Plan, your existing AI Credits allocation remains active through the end of the current paid billing cycle, after which the lower tier AI Credits allocation will apply and take effect to match your new billing plan.

3.2 Top-Up AI Credits

If you run out of AI Credits mid-cycle, your access to some AI features in FormsVault will be paused until the start of your next billing cycle or until you purchase standalone AI Credits. You can buy standalone “Top-Up” AI Credits packs at our then-current rates whenever you need.

4. No-Refunds Policy

4.1 No-Refunds and Subscription Adjustment Policy

Because our digital infrastructure, AI Credits, Forms Add-on functions, and features are available to use instantly upon payment, all cash payments and transactions made to FormsVault through Lemon Squeezy are final, non-refundable, and non-cancellable. This policy applies across all subscription tiers, automatic renewals, and individual AI Credits top-ups.

4.2 Distinction Between Refunds and Account Upgrades

While cash refunds are strictly prohibited, Lemon Squeezy may automatically calculate and apply a prorated balance adjustment when you perform an immediate tier or billing frequency upgrade. Any such balance calculation is strictly an automated offset against the purchase price of your upgraded tier and does not entitle you to cash payouts, bank returns, or withdrawal of funds.

4.3 Complete Exclusions

We do not provide cash refunds, bank chargebacks, or manual credit payouts under any circumstances, including but not limited to:

• Leaving a Subscription plan early or having partial unused months of service.
• Tier downgrades scheduled to take effect at the end of a billing cycle.
• Having unused or inactive team member seats or workspace license keys.
• Unused, leftover, or expired FormsVault AI Credits on your account profile.
• Forgetting or failing to cancel a subscription tier or downgrade request prior to its scheduled automatic renewal date.

5. Group Accounts and Workspace Team Invites

5.1 Workspace Management

If your subscription plan tier includes adding additional team seats or team workspace invites, the main account billing subscriber serves as the primary account owner. You assume operational oversight for all activities, data extractions, and configurations triggered within your active profile environment.

5.2 Shared Workspace Accountability

Any platform instructions executed or terms agreed to by an invited team member or guest collaborator within your workspace will be handled legally as an action authorized directly under your primary workspace account profile.

6. Our Community | Who Can Use FormsVault | Target Audience

6.1 Built for Professionals

FormsVault software is exclusively built, marketed, and structured as a professional business-to-business (B2B) productivity tool for independent consultants, freelancers, private coaches, corporate trainers, small-to-medium business groups (typically 2 to 100 person teams), and professional educators operating outside of traditional K-12 public or private school systems.

6.2 K-12 Disclaimer

FormsVault is not a K-12 educational student software application and is NOT directed at, marketed to, or intended for use by minors or K-12 students, and does not claim compliance with school district institutional IT frameworks. You must be at least eighteen (18) years of age to create an account, purchase subscription plans, or configure the Add-on. Minors and K-12 students are explicitly prohibited from setting up accounts or buying subscription plans.

6.3 Independent Classrooms and Audience Distribution

If you choose to distribute an output form or quiz generated by FormsVault to a minor audience, or use it within a private testing scenario, you acknowledge that FormsVault does not control or direct this distribution. You manage all local delivery parameters independently. By sharing these links, you provide an absolute warranty to us that you have secured any required student, participant, or parental permissions required by your local regional guidelines, and you accept sole responsibility for keeping your audience deployment fully compliant with all local privacy rules.

7. Distribution and Our Mutual Protection Agreement

7.1 Your Content, Your Control

When you build forms, surveys, deploy quizzes, or run automated analytics summaries over responses collected from live audiences, you assume absolute, sole and unshareable legal responsibility for the deployment, delivery, collection mechanics, compliance, and governance of those activities.

7.2 Consent Warranties

By distributing or sharing a generated Google Form or scanning an active response Google Sheets with our tools, the Customer expressly represents, warrants, and covenants to FormsVault that you have already secured explicit, valid and prior authorized legal consent from your respondents to track their browser window states via DOM (Document Object Model) signaling or run their responses using AI engines (such as Google Gemini). You acknowledge that you manage and retain exclusive custody of all resulting files, and you assume full responsibility for managing the visibility, sharing permissions, and security of these files.

7.3 Indemnification

To the maximum extent permitted by law, you explicitly agree to fully defend, indemnify, insulate, and hold completely harmless SALESVAULT TECHNOLOGIES PTE. LTD., FormsVault, its officers, directors, agents, and employees from and against any and all third-party claims, liabilities, financial losses, regulatory fines, data complaints, or legal expenses (including reasonable attorney and legal fees) arising out of or in any way connected to:

• Your distribution or deployment of any quiz, form, or tracking link built using the Add-on to any live audience, including the management of your respondents’ inputs.
• Any regulatory inquiries or data privacy complaints brought by local authorities or individuals regarding your processing of respondent data payloads under your regional privacy rules (such as US, British, European, Canadian, Australian, New Zealand or Singapore laws).
• Any structural guidelines or terms broken by a team member or collaborator you explicitly invited into your active account environment.

8. Intellectual Property and Content Rights

8.1 You Own Your Input

When you upload source files, PDFs, presentation slides, or website URLs into FormsVault to anchor our AI generation templates, you retain full and absolute ownership of your intellectual property. FormsVault does not claim any ownership rights over the text, files, or information you provide.

8.2 Grounded Generation License

You grant us a temporary, automated operational license strictly to process your uploaded materials through our in-memory AI infrastructure to generate your forms. Your files are treated as ephemeral inputs; they are parsed and immediately dropped from active memory upon execution, are not persistently saved on our servers, and are never used to train public machine learning or artificial intelligence models.

8.3 Your Warranty

You guarantee to us that you own the content you upload, or that you have the appropriate corporate copyrights, licenses, or legal permissions to use those materials as source references.

8.4 You Own Your Output

All output forms, surveys, quiz templates, and analytical results generated by the software are synchronized directly to your own managed Google Drive™ account. File access permissions, storage, and distribution are managed entirely within your own managed Google Drive™ account.

9. Respecting the Software

You may not access or use the Website or Add-on for any purpose other than that for which we make it available. Prohibited activities include, but are not limited to:

• Circumventing, disabling, or interfering with security-related features of the Website or Add-on, including tools configured to restrict the modification or extraction of underlying code frameworks.
• Tricking, defrauding, or misleading us or other users, especially in any attempt to learn sensitive account credentials.
• Using the Add-on to intentionally construct malicious phishing forms, deceptive consumer scams, or harvest data in violation of local laws.
• Reverse-engineering, decompiling, or attempting to discover the source code of any portion of the Add-on software layers.
• Utilizing automated scrapers, bots, or custom scripts to trigger excessive API loads across our active operational interfaces.

10. Understanding the Tech & Our Limits of Liability

10.1 As-Is Basis

We provide FormsVault on an "as-is" and "as-available" basis. Your use of our tool is completely voluntary. Because our Add-on plugs directly into external platforms, we cannot guarantee 100% uptime, nor can we be responsible for service blocks, text variations, or code errors caused by third-party systems, underlying Google Gemini AI models, Google Workspace™, Google Cloud™, or native Google Forms™ infrastructure changes.

10.2 The Absolute Liability Cap

Certain jurisdictions do not allow the total exclusion or waiver of local statutory consumer warranties. In those regions (including frameworks within Australia, the United Kingdom, the European Union, Canada, New Zealand, Singapore, Malaysia, the United Arab Emirates, Hong Kong and India), our software rules are modified to match those mandatory regional baselines. To the maximum extent permitted by your local laws, SALESVAULT TECHNOLOGIES PTE. LTD. and its team are not responsible for indirect, accidental, or unexpected business losses, lost revenue metrics, or data adjustments linked to your software use.

10.3 The Ultimate Financial Limit

In any event, our total cumulative financial liability to you under these terms for any claim whatsoever will never exceed the actual total amount you paid to FormsVault during the three (3) months right before the issue occurred.

11. Governing Law and Friendly Resolutions

11.1 Singapore Law

These Terms and your use of the Website and Add-on are governed by and construed in accordance with the laws of Singapore, without regard to conflict of law principles.

11.2 Direct Venues

Any operational claims, platform disagreements, or contract questions will be brought exclusively before the courts of Singapore.

11.3 Resolving Issues Individually

To help us solve any platform issues or questions quickly and smoothly, you and FormsVault agree that any official reviews or legal claims will be handled strictly on an individual basis. Both parties agree to resolve any platform matters directly rather than joining a collective, representative, or group-wide action.

12. Changing These Terms

We reserve the right, in our sole discretion, to make changes or modifications to these Terms at any time and for any reason. When we do, we will update the "Last Updated" date at the very top of this page. We encourage you to review this page occasionally to stay up to date.
